What is CRRT

CRRT, or continuous renal replacement therapy, is an advanced extracorporeal blood purification technology. This technology simulates human kidney function, removes harmful substances from the body and maintains homeostasis in the internal environment in a continuous and slow manner. It is mainly used in the treatment of patients with critical diseases such as acute kidney injury and multiple organ failure.

CRRT has various treatment modes, including continuous hemodialysis, continuous hemofiltration, etc. Which mode to choose mainly depends on the molecular weight of the product to be removed and the specific condition of the patient. During the treatment process, it is usually carried out in two modes: connecting arterial and venous lines or venous and venous lines, among which continuous veno-venous hemofiltration is more common.

When implementing CRRT, some key drugs, such as anticoagulants, need to be used to ensure the smooth progress of the treatment. Common anticoagulant drugs include unfractionated heparin, citrate, and argatroban. As a systemic anticoagulant, unfractionated heparin has the advantages of rich experience in use, easy monitoring and quick onset of effect; citrate is mainly used for extracorporeal anticoagulation. It blocks the coagulation process by chelating free calcium and has lower bleeding risk. risk; whereas argatroban is a synthetic low molecular weight direct thrombin inhibitor indicated for patients with heparin-induced thrombocytopenia.

CRRT plays an important role in a variety of clinical situations:

1. Acute renal failure, especially in patients with severe complications;

2 , severe infections, such as sepsis, CRRT can quickly and effectively remove inflammatory mediators;

3. In multiple organ failure, including liver failure, heart failure, etc., CRRT can maintain the homeostasis of the internal environment;

4. In poisoning situations, such as drug or poison poisoning, CRRT can quickly remove harmful substances from the body;

5. Other situations that require stable hemodynamics and continuous waste removal.

In short, CRRT plays an indispensable role in the treatment of critical illness with its unique advantages.
